I have beta 20 installed.

I was running beta 18 until today and have suffered for awhile now with the problem that the shell extension menu item: 'undo last operation from menu' was no longer working (undo always worked fine when editing files in the main application).

I thought upgrading might fix the bug but it didn't.

Finally I tried deleting a couple of my advanced 'expected filename format' patterns which I had defined and, to my great joy, it SOLVED THE PROBLEM!

Apparently, when the context menu list gets long, 'undo' stops working. I have many items in the list; can't tell you exactly how many items causes the problem.

I now have about 75 items in my 'expected filename format' list and more in the 'output filename format' list, as well as some macros too - and 'undo' is now working after I deleted two 'expected filename format' items.

FEATURE REQUEST (if you have time when fixing this bug):
Put the 'undo last operation from menu' at the TOP of the list instead of at the bottom. With a hundred items in my last, it takes a long time to scroll down to the 'undo' itme.

The shell extension undo feature is actually mostly what's holding back the 3.5 betas from becoming a stable non-beta version. It needs some time consuming fixes to work under all conditions. I don't really think is has to do with the number of menu items in your case, but rather which operation you performed last from the shell extension menu. For some operations, the shell extension undo feature just isn't supported in the current beta.
