OK, I was wrong. Having set tags using Windows Explorer doesn't effect this problem. Instead, it seems to be sample rate dependent.
The MP3 in question is an audio book, encoded with LAME (Poikosoft implementation,) VBR with the lowest quality settings: mode=mono, quality=very fast, variable bitrate quality level=9 (lowest), samplerate=22050.
Winamp File Info reports these specs:
Length: 45846 seconds 43kbit (VBR), 1755072 frames 22050Hz Mono
Even with no tags, MP3TagStudio won't perform a VBR header fix, failing with this error message:
Mp3 headers of of sync for file xxx, aborting! Repair failed, no changes made
MP3TagStudio doesn't seem to have a problem with MP3s encoded with the same settings, but at a 41000Hz samplerate.
Accurately repairing the VBR header is necessary here, since with an MP3 of this length, you could loose 5 minutes of playback at the file's end on an iPod if the VBR header is bad.
Any thoughts?
